homestay programの紙についてなんですが、署名のところに signature of witness name of witness （printed） address of witness occupation of witness とあるのですが、何を書けばいいのか教えてください。

"Name and Title of Signatory"の欄には、何を書けばよいでしょうか？ 現在、アメリカのメーカー宛てに契約書を書くところです。 4項目ありまして、 Name of Retailer Signature Name and Title of Signatory Date とあります。 Name of Retailer は販売店名だと思うのですが、 Name and Title of Singatory は何を書けばよいのでしょうか？ Signature は代表者名、 Date は署名した日付を書く、…で良いんですよね。

当方は株式会社ですが、インドの事務所から、英文の署名用の紙が送られてきました。 当方が、あることを依頼するときに出す委任状です。 この委任状には、日付以外に、(1)Signature、(2)Name、(3)Designation、(4)Official Seal という記入欄が上から下方向に、あります。 この中で、(1)Signature、と(2)Name、は、サインと、名前のタイプ打ち、ということだと思います。 それで質問ですが、(3)Designationは、役職(代表取締役社長など)のことでしょうか? また、(4)Official Seal とは、何でしょうか?

英語が全くの苦手で、基礎学力があれば簡単なことかとも思うのですが教えてください。 証明書の下の欄に下記のようにあります。 Ｙｏｕｒｓ ｔｒｕｌｙ, Ｔｉｔｌｅ： Ｎａｍｅ： Ｓｉｇｎａｔｕｒｅ： 会社の代表者（代表取締役）に証明＆サインをもらう書類なのですが、 会社名： 代表者名： サイン（署名）： ということでよいのでしょうか？ 上記でよい場合、表記の仕方は、 Ｔｉｔｌｅ→○○○○ Ｃｏ．，Ｌｔｄ Ｎａｍｅ→Company Executive ○○○○ ？ Managing Director○○○○？ Ｓｉｇｎａｔｕｒｅ→手書きでフルネーム でよいのでしょうか？ あと、、、加藤とか場合は、 「ＫＡＴＯ」「ＫＡＴＯＨ」と表記するのはどちらでもよいものなんでしょうか？ パスポートとかとあわせたりするのでしょうか？ 困っています。宜しくお願いたします。
