正解は ＞thatは単にensureとall of medical records are computerizedをつなぐための接続詞です。 確かに、 ＞to ensure that all of medical records are computerized within five years がわかりやすいでしょう。 前置詞句で始まると倒置みたいに言うこともあります。 まあ、どう呼ぼうがよくあることで、強調というかリズムというか。 In 2017, For Japanese people, 最近たまたま見かけた With ～, https://oshiete.goo.ne.jp/qa/9679388.html みたいに文で頭にくるのと同じで that 節内で within five years が先頭にくるだけのこと。 文の場合に、上の例のようにコンマがくるのが自然なように that 節でも自然です。 文頭の大文字にならないだけの違いで、 文頭のつもりで読めばいたって普通のことです。

この度はお時間を割いて頂き、ありがとうございます。 やはり、強調する効果があるのですか。 ところで、その後の調べで、出版社側の印刷ミスがあったことが発覚しました。 ホームページに掲載されていた正しい表記は以下の通りです（カッコ内が脱字） ensure that(,) whithin five years, all of medical records are computerized つまり、何のことは無い、ただの「挿入のコンマ」だったようです。

ざっと検索してみましたが、 ご紹介の ensure that within five years, all of ... 挿入的な ensure that, within five years, all of ... コンマなしの ensure that within five years all of ... いずれもあるようです。 コンマ一つで意味が変わるような場合もあれば、 結局、日本語の読点と同じで、人次第、気分次第、状況次第 のような、あいまいな部分もあります。 とにかく、前置詞句を前に出しているのは共通です。
