determining the degree of customer satisfactionとするとはっきりしませんか。訳せば QS-9000はサプライヤーが顧客満足度を決定するための文書化されたプロセスを要求しています。

#1さんのおっしゃるとおり、determine=「記録する」ではありません。「記録する」は、documentedにかかります。documentedの過去分詞（記録された）はそのまま受身で訳すと日本語として不自然なので、（記録する）と能動的に訳したのでしょう。 私がもし拙訳を許されるなら、 （直訳） QS-9000は、売り手が顧客満足度を決定する書式化されたプロセスを持つことを要求している。 （分かりやすい日本語） QS-9000では、売り手が顧客満足度の査定基準を書式で設定するように定めている。 つまり、お客さんがどの程度満足したか、個人の主観が入らないように、明文化しておくということでしょうか。supplierは、vendorともいい、商売では売り手のことです。

to have a documented process のところが「記録する」という意味でしょう。 determine は日本語としておかしいと思い、訳さなかったのでしょう。

この一文だけだと、「顧客満足度を決定するためのプロセスを定義した文書があれば良い」とも読めますが、 前後の文脈、あるいはQS-9000だかの実態とかから、そう訳されたのでは。

determineを記録すると訳しているわけではありません。 直訳すれば、 「QS-9000は供給者に顧客の満足度を測定するための記録過程を要求する。」 となります。 これを、こなれた日本語にしたらそういう文になったものと思われます。
