学校で習ったことが間違いという訳ではなく、 「２番目の～」という意味ならば、普通は、 特定のものになるので、theが付きます。 (この場合も例外皆無という訳ではありませんが、 原則は、こういうことです) ところが「second」には、その意味から派生して、 「もう１つの」とか「次の」、さらには 「劣った」(１番目に負けている)という意味で 使われることがあり、そういうときには、 必ず、theが付くとは限っていません。 この場合の「second opinion」は、 最初のお医者さんの診断に対して、別のお医者さんの 「もう１つの」診断、と言う意味で、自動的に、 特定のものを指す表現ではありませんから、 aがついている、ということです。ついでにいうと、 この場合なら、いつもaという訳ではなく、 すでに得られた特定の「second opinion」なら、 勿論、theが付きます。

基本的に、「もうひとつの」と言う意味での second はあっていますが、a second opinion と second opinion の違いなどありませんよ。 なぜなら、無冠詞の second opinion というようなものはあり得ないからです。 opinion が無冠詞になるのは、public opinion など総称する場合だけです。

＃２です。補足です。 ＞＞ではa second opinion と何も付かない唯のsecond opinionの違いがちょっと分かりかねます。よろしければその件についてもご説明頂けたら光栄です。宜しくお願い致します。 second の代わりに他の形容詞、たとえば reasonable を代用したと同じ状況です、その場合 a reasonable opinion と、不定冠詞 a がつきます。 a second opinion と不定冠詞がつく場合と、冠詞の無い second opinion は「別の医者の意見」という一般的なものの違いで、a big dog 「一匹の大きな犬」と big bog 「大きな犬」の違いと同じです。

a second opinion 「不特定の(誰か)別の医師の診断」 the second opinion 「特定の別の医師の診断」 冠詞によって意味内容が別のものになってくるのです。

40代男性です。先日、A病院で初期の腎臓癌と診断されました。 主治医の先生から治療の説明がありましたが、Ａ病院では腹腔鏡手術には対応しておらず、開放手術となるとのことでした。その他、数か月様子見する等の選択肢を提示され、どうするか考えてほしいと言われました。セカンドオピニオンも、むしろお勧めするとのことでした。 自宅で色々と調べたところ、やはり開放手術は身体への負担が大きいようで、腹腔鏡手術を受けたいと思いました。腎臓癌の治療で有名なB病院は腹腔鏡手術の実績があるようでしたので、翌日B病院へのセカンドオピニオンを申し込みました。まだ紹介状はいただけていません。 ここで疑問なのですが、B病院での手術を希望している場合、セカンドオピニオンは意味があるのでしょうか。転院がスムーズに進むか心配です。それとも、どうしてもB病院で手術を受けたいのであれば（あまり良くないかもしれませんが）、検査をやり直すのを承知で、B病院の新規外来を受診してしまったほうがよいのでしょうか。 初期の腎臓癌は進行が遅いらしく、そういう意味では時間的余裕があるようですが、やはり怖いのでできるだけ短期間でB病院の手術を受けたいと思っています。もちろん、有名な病院なので予約が先々まで埋まっていることは予想されますが、それは承知の上での話です。
