This route is suggested as an adjunct for patients with persistent or severe CDAD, severe ileus, or fulminant colitis. The medication can be given at dosing intervals of 4 to 12 hours, with doses of 2 to 3 g/d, by using an 18F Foley catheter or a soft 6F pigtail catheter. The risk of bowel perforation, inherent any time an enema is given, mandates particular vigilance for these patients. Ideally, the enema should be retained for 60 minutes. Vancomycin can also be given via colostomy, ileostomy, or during colonoscopy. Oral rifampin has been given with metronidazole as an adjunct therapy. A prospective, randomized, single-blind study of 39 patients was conducted to compare the efficacy of metronidazole therapy alone with that of metronidazole plus rifampin. The researchers could not demonstrate a therapeutic benefit of this drug combination and halted their study early, citing study futility and urging better treatment protocols. More promising results are suggested by a study in which oral metronidazole was compared with nitazoxanide. A nitrothiazolide, nitazoxanide works by blocking anaerobic metabolism and is used to treat intestinal infections caused by the protozoa Cryptosporidium and Giardia. Patients receiving nitazoxanide were given 500 mg every 12 hours for 7 or 10 days. Those receiving metronidazole received 250 mg every 6 hours for 10 days. For both groups, the time of resolution of their signs and symptoms, clinical response after 7 days of treatment, and sustained response after 31 days were determined. Initial data suggest similar effectiveness for metronidazole and nitazoxanide. Further studies are being designed to compare this new medication with vancomycin.

<Antibiotics> The first treatment for CDAD is cessation of the suspected causative antibiotic. Although stopping the antibiotic may be effective, the reason the antibiotic was prescribed in the first place should be considered, and monitoring for recurrence or worsening of the underlying infection is essential. Oral metronidazole, 250 to 500 mg 4 times a day for 7 to 14 days, is generally recommended as an initial treatment.29 Metronidazole can also be given intravenously. Administered by either route, the same dosage provides bactericidal levels of the drug in the bowel lumen. Although equally effective as metronidazole when given orally, vancomycin, standard dosage 1 g/d for 10 days, is more expensive, and is therefore reserved for patients who can not tolerate metronidazole, are pregnant or breastfeeding, or have severe cases of CDAD.Alternative vancomycin dosing strategies may be effective for treatment of recurrent CDAD. A pulsed dosing regimen, intermittent delivery of between 125 and 500 mg every 3 days, and tapered dosing, administering the antibiotic over long periods at decreasing doses, may in some instances effectively eradicate vegetative C difficile cells. In addition to its greater cost, a disadvantage of vancomycin is the occurrence of vancomycin-resistant strains. Intraluminal concentrations of vancomycin cannot be achieved by intravenous infusion as they can with metronidazole. However, vancomycin can be given via a nasogastric tube or as an enema. Instilling vancomycin as an enema can be challenging, particularly when a patient is experiencing diarrhea.

The latest study involved 40 patients infected with P. falciparum in Pailin, and another 40 in Wang Pha, in northwest Thailand2. Half of each group were given just artesunate, and the other half the dual artesunate–mefloquine therapy. The Cambodian group took almost twice as long to clear the parasite from their body as the Thai group: 84 hours compared with 48 hours. Usually artemisinin drugs clear the parasite within three days, says Nick Day, director of the Mahidol Oxford research unit in Bangkok and co-author of the study, published in the New England Journal of Medicine. Infection broke out again for six of the twenty patients taking just artesunate in Cambodia, compared with two patients in Thailand. With the combined therapy, it recurred in only one of the twenty cases in each group. The World Health Organization recommends that artemisinin-based drugs be given only in combination, to delay the onset of resistance. But Day explains that the single-dose groups were crucial in their controlled study to show that there was growing resistance to artesunate, and not to mefloquine alone. The emergence of artemisinin-resistant malaria in Cambodia is especially worrying, because previous drug-resistant strains of malaria have sprung from there. "This is a wake-up call," says Day. "We have got reduced susceptibility to the drugs in an area where we've seen resistance emerge before." Given time, the parasite will only become more resistant to the drugs, he adds.

A soluble, high-molecular-weight, anionic polymer, tolevamer works by binding C difficile toxins A and B. Suggested for use with mild to moderately severe CDAD, tolevamer is given orally 3 times daily for a total of 3 to 6 g per day. Initial trial results indicted that the polymer was fairly well tolerated, provided resolution of signs and symptoms in 67% of patients, and was associated with a lower incidence of recurrence than was vancomycin. Resolution of diarrhea was comparable to that achieved with vancomycin; however, tolevamer is also associated with an increased risk of hypokalemia. <Immunoglobulin> Immunoglobulin has been proposed as a treatment for intractable and severe cases of CDAD. In one study, patients were given 1 to 6 doses, ranging between 300 and 500 mg/kg. Of 5 patients with refractory disease, 3 had resolution of signs and symptoms within 11 days. Although this treatment looks promising, immunoglobulin is expensive and in short supply. <Probiotics> Probiotics are live organisms meant to treat particular diseases, including Helicobacter pylori infections, acute gastroenteritis, and antibiotic-associated diarrhea, including CDAD. The most commonly used probiotic agents produce lactic acid and are present in normal microflora. Examples include Saccharomyces boulardii, Lactobacillus acidophilus, and Lactobacillus plantarum. Many studies of probiotics have been done. Sullivan and Nord suggest that although S boulardii was somewhat effective, particularly in preventing recurrent CDAD, further studies are needed. Dendukuri et al similarly determined that evidence was insufficient to recommend the routine clinical use of probiotics to treat CDAD in adults.

“Unlike previous investigations, we were able to assess association of consumption of caffeinated and non-caffeinated beverages, and we identify caffeine as the most likely candidate of any putative protective effect of coffee,” said lead researcher Michel Lucas, research fellow in the Department of Nutrition at HSPH. The authors reviewed data from three large U.S. studies and found that the risk of suicide for adults who drank two to four cups of caffeinated coffee per day was about half that of those who drank decaffeinated coffee or very little or no coffee. Caffeine not only stimulates the central nervous system but may act as a mild antidepressant by boosting production of certain neurotransmitters in the brain, including serotonin, dopamine, and noradrenaline. This could explain the lower risk of depression among coffee drinkers that had been found in past epidemiological studies, the researchers reported. In the new study, researchers examined data on 43,599 men enrolled in the Health Professionals Follow-Up Study (HPFS) (1988–2008), 73,820 women in the Nurses’ Health Study (NHS) (1992–2008), and 91,005 women in the Nurses’ Health Study II (NHSII) (1993–2007). Caffeine, coffee, and decaffeinated coffee intake was assessed every four years by questionnaires. Caffeine consumption was calculated from coffee and other sources, including tea, caffeinated soft drinks, and chocolate. However, coffee was the major caffeine source — 80 percent for NHS, 71 percent for NHS II, and 79 percent for HPFS. Among the participants in the three studies, there were 277 deaths from suicide. In spite of the findings, the authors do not recommend that depressed adults increase caffeine consumption, because most individuals adjust their caffeine intake to an optimal level for them and an increase could result in unpleasant side effects. “Overall, our results suggest that there is little further benefit for consumption above two to three cups/day or 400 mg of caffeine/day,” the authors wrote. The researchers did not observe any major difference in risk between those who drank two to three cups of coffee per day and those who had four or more cups a day, most likely due to the small number of suicide cases in these categories. However, in a previous HSPH coffee-depression study published in JAMA Internal Medicine, the investigators observed a maximal effect among those who drank four or more cups per day. One large Finnish study showed a higher risk of suicide among people drinking eight or nine cups per day. Few participants in the two HSPH studies drank such large amounts of coffee, so the studies did not address the impact of six or more cups of coffee per day.

The formulation of polisies and procedures can be defined as either strategic or tactical. Strategic decisions are concerned with the long-term health of the enterprise. Tactical decisions deal more with the day-to-day activities necessary for efficient and smooth operations. But decisions, either tactical or strategic, usually require implementation by an allocation or reallocation of resources-funds, equipment, or personnel. Strategic plans can be formulated from below, but normally the implementation of such proposals requires the resources which only the general office can provide. Within the broad policy lines laid down by that office and with the resources it allocates, the executives at the lower levels carry out tactical decisions.

To me it was like and old and neglected building, I wanted to restore it to its former glory. The chart was like deciphering an ancient book of signs and symbols and translating them for you into language you can understand.
