うまく日本語訳にできない、文構造がわからない

こんにちは＾＾ 私は英英辞典を使っているのですが、utterlyという単語を引いたところ、 used especially to emphasize that something was very bad, or that a feeling was very strong. と書いてありました。 usedはどんな働きをしているのかよくわかりません。 そもそもいきなりusedとはじまっているのはどういうことなのでしょう。え？いきなりなんで過去形？って感じでして。。。 あともう一つ質問があります。 insistという単語を引いたところ、 to demand that something should happen. と書いてありました。 これは、～を要求すること、と文全体が名詞になっているわけですが、 something should happen　のところがわかりません。 このsomethingはいったいなんなのか、 また、should の後ろは原形のはずなのにhappenがなぜ使われているのか。 以上が質問の内容です。 回答よろしくお願いします。

こんにちは！ 　英英辞典を使われると、それ自体が１つの立派な勉強になりますね！ （１）used especially to emphasize that something was very bad, or that a feeling was very strong 　●こういうふうに、過去分詞（過去形ではありません）で始っているのものは、用法の説明をしているのです。 　【The word "utterly" is】used especially to emphasize ... の【】部分が省略された形です。～～の時（文脈）で使われる、の意味です。日本語では「まったく」と覚えますので、 　× The scenery was utterly wonderful. なんてやりかねませんが、これを読むと否定的な場合に使われることが分かります。 　ちなみに、a feeling was very strongというのも、I have a strong feeling about ～という言い方を下敷きにしていまして、ホントにひどい！といった感情を表しています。ですので 　Mr Go_urn, your essay is utterly ludicrous, and I have come to embrace the notion that my tutoring was utterly futile! 　なんて調子で使う言葉ということですね！ （２）insist: to demand that something should happen 　● demand のように要求を表す動詞の目的語になる that 節の中では、仮定法現在形＝原形を使うというのはご存じです。 　面白かったのは、私の世代は、that 節の中では should を入れるという文法を習い、初めて原形に遭遇したとき面食らったことがありまして、ちょうど逆になっているわけですね。 　この辞書はイギリス系ではないでしょうか。イギリス英語では、should を入れることが多いと思います。アメリカ英語も入れる場合もありますが、仮定法現在形＝原形が圧倒的に多いと思います。 　以上、ご参考になれば幸いです！

詳しい回答ありがとうございます！ the word utterly isが省かれて書いてあるわけですね！ 英英辞典の説明からすると、utterlyはなにか悪いこととかマイナスのことを強調するときに使うわけですね♪ 英英辞典は今日使い始めたばっかりなのでちょっと質問させてもらいました＾＾

最初のご質問は、英英辞典の "utterly" の説明なので、"'utterly' is used especially to emphasize that something was very bad, or that a feeling was very strong."、つまり、「'utterly' は～のために使われる」という意味の "used"です。 ２番目のご質問の "something"は、"someone"と共に、特定する必要がないものとして、英英辞典でよく使われます。文字通り「何か」「誰か」という意味で、"insist"の場合は「何かが起こるべきであると要求すること」ということになります。なお、"happen" は原形なので、何か勘違いをされているのではないかと思います。
