質問

Nowadays, fortunately, most cataracts can be cured by a simple oparation in which the lens is removed and replaced with an artificial one.（現在では幸い、大部分の白内障は、水晶体を取り出して人工のものと交換する簡単な手術で治すことが出来る）

この文章で１つ疑問に思ったことがあるのですが、なぜwhich の前にin が付いているのでしょうか？
この部分が良く分からないので、文法の解説をしていただきたいです。（in がなくても意味が通じるのではと自分では感じました。）

(1) The lens is removed and replaced with an artificial one in a simple operation.
という文が前提です。
(2) Nowadays, fortunately, most cataracts can be cured by a simple operation.
この文のoperationを先行詞として、関係代名詞の節で修飾します。
そのとき、(1) のin a simple operationの部分を関係詞にします。
in a single operation = in whichと置き換えて
それを前に持っていきます。
Nowadays, fortunately, most cataracts can be cured by a simple oparation in which the lens is removed and replaced with an artificial one.となります。

もっと単純な例で説明します。
This is a hotel.
I stayed in the hotel three years ago.
この２つの文を関係詞で結ぶとき。
This is a hotel which I stayed three years ago.
としますと間違いです。
This is a hotel in which I stayed three years ago.
または
This is a hotel which I stayed in three years ago.
としなければなりません。
inをとってしまうと
I stayed the hotel three years ago.
とやっていることになり、自動詞のstayのあとに目的語が来る形になるので、文法的に間違っています。ご質問の文も同様です。

なお、この場合は関係副詞を使えば
This is a hotel where I stayed three years ago. となり、これも正しい英文です。
この英文は
I stayed there three years ago. のthereという副詞がwhereという関係副詞になったわけです。
thereは名詞でなく副詞で、whereも（綴りが似てますね）副詞です。
ちなみにthere=in the hotelですね。
where = in which ということになりますね。
