サインが必要なのか、どこにするのか教えてください。

On _July _11__ , 2014, before me personally appeared _Taro Urashima___ , to me personally known に貴兄の名前が入ると思う。サインになるのかタイプなのかわからない。多分、後者で、貴兄がサインをする必要はないと思う。
