otherwise は基本，副詞です。 「もしそうでなけれれば」という場合は ; otherwise he would have ～のような接続詞的に用いることが多いのですが， 接続副詞と呼ばれ，接続詞でなく副詞には違いありません。 だから，普通に副詞として助動詞と動詞の間にきて「そうでなければ～しないだろう」 と動詞を修飾することができます。
