貴方のご理解すべて正しいと思います。（２）の？も？でこんな言い方はしません。これはたまたまhaveを使っているからで、 I know a friend who lives in Canada. 『カナダに住んでいる友達を（とりあえずは）一人知っている） I know the friend who lives in Canada 『カナダの住んでいるというその友達を知っている』 というようになると思います。
